Issue: QRDA files created in OfficeMate 12.0.3 SP8 are not accepted on QualityNet by CMS. The files may not be accepted due to the following issues:
- Missing provider NPI and TIN (EIN) numbers (error code 71170)
- A value contains more than six digits to the right of the decimal (error code 711295)
- The reporting parameters section does not contain exactly one code or value (error code 23-19229 or 711292)
- Data II types do not have the correct root, null flavor, or extension values (error code validate_II)
Resolution: If you are submitting Level II codes via insurance claims, Medicare already has your PQRS data. For meaningful use CQMs, run the calculator for each CQM and attest to meaningful use using the numerator, denominator, and percentage reported in the calculator.

Note: The last day for PQRS claims-based submission was February 28, 2017. The last day for PQRS qualified registry submissions is March 31, 2017.
