Abstract:
If a staff member or provider has forgotten their provider portal password at https://eyefinityew.com, follow the instructions below to reset it:
- From within the provider portal at https://eyefinityew.com, click the Forgot password link above the Password text box.
- Enter the staff member or provider's Username and click Send New Password.
Note: If the staff member or provider does not remember the username, ask the Secure Messaging Portal Admin who set up the Portal to verify the username within the provider portal by logging in, clicking Menu, clicking Admin, selecting Users, selecting the user with the forgotten username, and viewing the user's Username. Do not change the username, as it will adversely affect meaningful use counts!
