Note: The information in this article only applies to paper CMS 1500 insurance claims in OfficeMate 8.0 and below!
Medicaid requires that only NPI numbers (and no legacy numbers) are recorded on CMS 1500 forms. Medicaid is also requesting that the taxonomy code with a ZZ qualifier is recorded in place of legacy numbers on CMS 1500 forms.
To automatically populate box 17A and box 32B on the CMS 1500 form with the taxonomy code and ZZ qualifier, follow the instructions below in OfficeMate:
- In OfficeMate 8.0 or below, click Setup and select Business Names.
- Select Provider Taxonomy from the Qualifier (32b) drop-down menu.
- Type the taxonomy code in the Facility ID (32b) text box.
- Click the Referring Dr. tab.
- Select the referring doctor from the Select Referring Dr. drop-down menu.
- Select Provider Taxonomy from the Qualifier (17a) drop-down menu.
- Type the taxonomy code in the Other ID (17a) text box.
- If you have multiple places of service, click the Place of Service tab and follow the instructions below.
- Select a place of service from the Select Place of Service drop-down menu.
- Select Provider Taxonomy from the Qualifier (32b) drop-down menu.
- Type the taxonomy code in the Facility ID (32b) text box.
- Click OK.
OfficeMate cannot automatically populate box 33B and box 24J with the ZZ qualifier because they are connected to the insurance type. You can, however, automatically populate the taxonomy number and then manually record the ZZ qualifier. To accomplish this task, follow
